I was in the same boat (my levels went from something like 200 -> 260), and my doctor did not take me off of Accutane, saying it should go back down once I’m off of it. Similar to you, I’m an avid runner/weightlifter every day and just have bad genetic luck for scary cholesterol. I’ve been taking fish oil and also added in plant sterols about a month ago, which did lower my cholesterol about 9 points this past month.

Mine was higher than that and they allowed me to keep taking.
