I think it was a “smart” or self-guided anti-ship missile and it locked on to the ship that had placed the target ship because it had not vacated the hot zone or danger area.
From what I’ve seen they’re a very interesting navy, in an attempt to roast a British destroyer in international waters off the coast of Iran they tried to tell the captain of said destroyer that his English was awful and that’s why they kept antagonising the destroyer

It's not very clear where that number of injuries was derived, but probably fair to assume at least that all of the dead were Konarak shipmates! I think in situations like this early numbers are often unreliable, even in first world country tragedies, and with this one international politics, tensions, and good old embarrassment about this event might hinder clear reporting.
It says it was overhauled in 2018, I’m thinking they may have had to expand the crew to handle maintenance and operations needs. Still, adding almost double the crew still seems like a lot. It may have been a naval boarding exercise and it was carrying a boarding party.

The *Noor* is a Chinese C802 missile (export version) that was reverse-engineered by the Iranians. The C802 has a listed maximum speed of 0.9 Mach, or somewhere in the neighborhood of 700mph.
The *Noor* has reportedly had its range improved from ~20 miles to about ~80. There's a chance it's slower than the C802, but probably not by a huge amount.
Some Russian anti-ship missiles are reportedly capable of Mach 8 (6000 mph, approximately), though.
Very, not sure if these ones were designed to stay above surface and then climb and dive directly down on the target or just on surface to avoid phalanx countermeasures etc.
The ‘crew’ size usually refers to the minimum number of people required to man the ship to carry out its operations. Often, there would be more than the minimum, e.g. crew under training. Given the nature of this exercise (missiles are very expensive), some possibilities include embarking additional military staff to to evaluate the missile, or evaluate the conduct of the exercise itself (although I think the more important officers would have been on the frigate itself than the Konarak)
The Dutch built the ships originally as just logistics tenders and patrol boats. Iran has up gunned them into budget missile boats with targeting radars and bolt-on-deck missile launchers. That means more crew to run and maintain the weapons systems. And if they were deploying targets, probably some crew handling that as well

The ship apparently was laying out targets for the missile ship and failed to get adequately far from the target. Now it’s not clear if this was human error (perhaps the missile ship fired too quickly?) or something kind of mechanical error (perhaps it had power failure while trying to leave the area)
The missile has active radar homing, so they probably just accidentally targeted the support ship. It would be pretty hard to “accidentally” make that shot without the missile’s own guidance.

USS VINCENNES shotdown an Iranian civilian airliner in the early 2000s because it's targeting radar inncorrectly showed it decending towards the ship rather than climbing well above it. the ship's lookouts frantically looking at sea level were surprised and then horrified to watch their missile swiftly ascend towards an obvious airliner and blow it out of the sky with no possible survivors.
This was 20 years ago, and yes it did happen but was literally ages ago and only happened once.
