A ring that tells you if you are being watched by a threat. Don't even accumulate insanity points, just give him +5 to initiative and tell him whenever he's being watched by an invisible/hidden hostile creature. Start a campaign focused around doppelgangers/rogues/assassins. Then after they start relying on its effects have it begin glowing for no reason, "oh the ring started glowing, you know who was outside your room? The innkeeper, they were 'mopping'." But then they actually were just mopping and the ring was lying to you about the danger.

I would just put it as detecting hidden or invisible creatures with, say, 60ft and leave out the word hostile. Use the adventure to imply the creatures are hostile, and get liberal with your definition of "hidden creature". Like, if they're in a basement, there might be hidden rats. Or, the ring glows and gets warm when being scryed. It gets scryed often at dramatic moments. Truth is, the ring isn't even cursed. Someone is just looking for it because they lost it.

Maybe it never lies, maybe it only warns his wearer when itself is in danger. Danger for the Ring would be "not beeing worn" so anything that would harm its wearer or uncover its malice. Could be, that the ring was made by a fey bard, that uses it to spy on people to find and steal items of high emotional value to power their "mood [instrument]"

A magic gold tooth or mouth piece, that allows user to taste lies, deceit or bad intent around them. It however lies and progressively begins speaking for the user, removing their vocal inhibitions. They begin voicing their immediate thoughts regardless of whether they mean it. This begins unwinding their mind naturally.

Pick a 'mad' character from media, favored by both you and the player. Develop an item around the concept. Ex. Heath Ledger's Joker. Ex2. Small notebook. Words inscribed on the cover: Some men want to watch the world burn. Effect1: Sometimes, DM forces PC to remember the words. Effect2: the notebook records any atrocities committed by the player. Maybe grants tHP? Restores a low level spell slot? A minor boon, enough to guide their actions, but not to dictate them.
