Yea this looks exactly like the flaky spots I had on my scalp that I tried to "self-treat" and resulted in a nice 1-inch diameter bald spot. My derm suspected psoriasis but could also have been eczema, the treatment was the same either way which was topical steroids designed for the scalp. Cleared up ridiculously fast and I felt really silly for waiting so long and had to deal with regrowing that hair because of it.

I used tgel, ( the brown one) and it worked for a while but then stopped working. So I changed to a very mild V05 shampoo. It was OK but not great. After that I used a tea tree shampoo. It was the same as the V05. Went back to gel, and it worked like a dream.

Oh no! I have psoriatic arthritis and apparently I wasn’t absorbing the methotrexate when I took it orally and they just switched me to methotrexate shots so I’m hoping that helps. My hands are less swollen today than they have been so that’s good news!

Try the walgreens brand ‘original t-gel shampoo’ (if you have one near you)! a man on tiktok, philfromnewyork, posted about neutrogena’s being discontinued and he tried out that brand and said it worked great, he called it the holy grail

Omg me too!! I LOVE picking things and I didn’t realize I picked too much of my hair out with the skin flakes (sorry sounds disgusting I know lol) so I had a bald spot on top of my head. Can you share what those were? I have seborrheic dermatitis and my derm gave me a minoxidil spray to regrow hair. I never used it except for a week since I was worried about the side effects and I do have health scares so I stopped using it.
I recommend using Nizarol - it helped me a ton with my psoriasis, triggered by stress. I started by using it every time I showered (leaving it on for 3-5 mins). Once things were under control, I started using it once a week and then every few weeks.
Any dandruff shampoo can dry out your hair, so I would say use a hair mask more than often. You can also use a glycolic acid exfoliater (even the ordinary one for $7 - seems recommend that) to help with some exfoliation maybe 1-2 times per month.
Oh so Nizoral actually helps with Psoriasis instead of just dandruff?? I'm gonna have my husband try that. He gets *really* bad Psoriasis on his head and face. The only thing that works is a special cream he gets from the doctor, but we can't use it in his hair. So he's been using a mixture of shampoo and baking soda.
The shampoo and baking soda actually works really well but he has to use it at least every other day, or we start to see it coming back already. No matter how long he's been using it for.
At the very least, you should switch to an anti-danduff shampoo that targets seborrheic dermatitis. This would be something like selsun blue or head & shoulders clinical (which use selenium sulfide as an active ingredient).
It’s def seborrheic dermatitis, my husband had this. You should go to your doctor but T-gel worked really well for myself (left it sit a white before you rinse)
I agree! I switch back and forth between T-gel and Nizoral shampoo. I can't use anything else, or even dry shampoo even once without flake city returning.
Came to say same - and also don’t sit on it, because come to find out it can spread to your face, and even your ears. Have had a chronic flare up in both ears for months now (stress) and have a freaking bald spot in my eyebrow.
I used to exclusively use Tgel because it was the only thing that worked for me. My stylist had me try Lanza scalp balancing shampoo and conditioner. It works even better and doesn't smell like tar lol
Yes. Please don’t pick it. Those scales can hold bacteria, which can cause infection, creating damaged follicles and even scar tissue. There’s your bald patch.
Also, you don’t want that stuff under your fingernails and then you go eat a sandwich.
If you’re not going to the doctor, start with Selsun Blue or T-Gel and see what happens after using it a few times a week. If it doesn’t clear up then you absolutely need to because it possibly could be fungal, which can spread. Even if not, it’s not good for you.
Plus, it’s just nasty. It smells like flopsweat and will always be present in your hair, even when you’re clean. I’ve seen clients with it and I usually recommend a dermatologist. Nine times out of ten, I can tell they won’t go and then I awkwardly have to explain that they can’t come back until we can confirm it’s not fungal or something.
I don’t judge because it can happen to anyone, but others will, like at your job or on a date. It’s just best to get it sorted out quickly before it gets worse. Left unattended, it very possibly can and you’ll be kicking yourself about it.
Don’t pick it. I know, I know, I know, but go see someone. Most of these things don’t have actually long lasting hair loss.
There are so many things that can make this happen so it’s best to see a doctor. If only yo dave you time and the chore of dealing with flakes and plaques.
Please listen and read carefully: This is only going to go away and be treated by a dermatologist. It is most likely plaque psoriasis. This will not go away with a cream, a shampoo or a home remedy as what is causing it is caused by your immune system and it can and will get worse and more painful. Please seek out a licensed dermatologist as soon as possible, as this could be the sign of something more serious happening to your body.
This was one of my main symptoms when I was little for my Psoriatic Arthritis. If the meds that are available now we're available then, I could've had a much easier time, and there's be much less damage now. Definitely see a dermatologist and rheumatologist ASAP.
Absolutely agree. I struggled with home “treatments” for years before I saw a dermatologist and after 1 appointment and prescription cream/shampoo it cleared up within a week.
I started getting seborrheic dermatitis on my scalp and face as a kid and my family went to a general doctor and we were told it was ringworm so we “treated” it with otc fungal cream for a year. During that year it got worse and worse to the point where my skin constantly raw and bleeding, so we eventually went to a dermatologist who diagnosed it and prescribed a cream and shampoo. After one use there was immediate improvements and it cleared up within a week.
The derm appointment was literally less than 15 min and could have solved years of suffering if we had went to the dermatologist first!
I have psoriasis…the dermatologist needs to test it. I have gone to dr that just diagnosed it by vision …do not let them do that. It took years to control. Psoriasis is an autoimmune disorder …I have gotten sick over the years and it spread to my entire body. They first told me it was dermatitis years of steroids and crap that does not work. Yours looks more on the dermatitis side…mine is just white and red under it. Those shampoos are good at clearing once in a while like once a week . That is it. They dry your hair out and coal tar…which is tgel…causes cancer. This is true information. I have to take an injection of skirizzi and my scalp still has it some but it’s is controlled. Nizoral is for anti fungal dandruff etc…just find out what it is before slapping crap on your head. I have had psoriasis for 30 years … you are
Lucky they now have medication that works pretty well….they did not when I was a child. Going to the tanning bed once a week probably heals it the most though. UV lights do something to it…like tempo kill it. So I started tanning at a really young age…living on the beach was pretty much the best thing…along with prescription medicine. Nothing over the counter has ever worked for me. There is a good chance you do have dermatitis though and they have different scripts for that. They also have a new script for some types of eczema that supposedly clears it up amazingly fast.
This looks like, seborrheic eczema, but you must go to your dermatologist/trichologist.
There are many ways of fixing it, so don’t worry.
Just don’t google things. They go a little out of hand holistic. Go to a doctor.

But I highly recommend using Nizoral. My hubs scalp used to look like that and Nizoral helped clear it dramatically!
You really need to see a doctor. They can give you a prescription shampoo that will resolve this very quickly. Seriously, you’ll be kicking yourself for not going earlier.
Unrelated but I’m curious why you only use shampoo? Wouldn’t that just make your scalp and hair dryer and ultimately more greasy?

You might be allergic to something in your shampoo. Or if you're extra lucky, you might even be allergic to something in your water.
I'll echo the other comments to visit a dermatologist. I know that can take a while, so in the meantime, I'd switch to using Vanicream's Free and Clear shampoo. It has the fewest number of ingredients and is made to be as skin-friendly as possible. My derm recommended it to me when I first started by eczema \~journey\~
If you tried using Selsun Blue or dandruff shampoo like head and shoulders and it’s still not working, you might have dermatitis or something similar. I have the same problem. The doctor recommended Nizoral and it worked for me. It’s over the counter anti-dandruff shampoo I think? You can get it at a pharmacy. But if that doesn’t work, they prescribe steroids for the scalp. I’d recommend going to the doctor and checking this stuff out.
Great recommendation! Just to piggyback off of this… I had something similar and my derm recommended Selsun Blue because it has a different active ingredient than Head and Shoulders. When you apply, let it sit for 5-10 mins approx 3x/week. It needs to sit on the scalp to kill the fungal growth. Nizoral will work the same way, too. I tried to keep it off of the ends of my hair because it’s drying. Doesn’t smell the greatest but it’ll do the trick. Also, dry your hair with a blow dryer. Keeping the scalp dry will keep overgrowth at bay. Might save you a co-pay the dermatologist… but if that doesn’t work, see a doc!
Coconut oil, sleeping with wet/coated hair, scrubbing etc will make it worse.
Great tips! Yeah Selsun blue is really good too! It usually works for me as well. For some reason though, when I’m really stressed, it doesn’t do the trick completely :/
Also yes, OP don’t use coconut oil! I saw some people suggest that. It will most likely just make it worse and feed the fungal infection if there is one.
I really think you should go to a doctor or a dermatologist. You could even send in pictures via teladoc if you insurance covers virtual visits. Your doctor will be able to prescribe you something that clears up your current flare up and will help you with what products to use and what to avoid to prevent future flare ups.
My husband's scalp looks like this and has refused to go to the derm as recommended by the doctor. It gets itchy and he scratches at it so hard it leaves bloody furrows in his scalp. Sometimes I'll lightly scratch/peel it for him so he doesn't carve up his head like that. Really needs to just go to the derm as you should. It has only gotten worse over the years for him.
Tell him he needs to do it. Point blank. You scratching the plaques if it is psoriasis likely makes it worse. He needs to do for himself and for you. If he has psoriasis then he feels the burning and discomfort.
He needs to get this settled because with a lot of these things the longer you let it go the worst it gets.
It’s important to get this under control. He will be able to feel so much better.
This kinda looks like pityriasis amiantacea. If you try to remove the build up, is it pulling your hair at the same time (is it stiking to the shaft of the hair near the scalp and won’t go away)? I had a similar condition occur to me a couple of years ago and I got rid of it using Nizoral once a week and alternating with Neutrogena T Gel. However what I had might be completely different from you, I never went to a dermatologist so I’m not sure what it was. As other people recommended, it would be better to consult a dermatologist since they could give an accurate diagnosis and prescription.
Seb derm or psoriasis. Get thee to a dermatologist! It’s probably easily treatable with the right prescription and instructions.

My 10 year old son had this. He’d shower and after I would try combing them out of his hair and scalp. The information I found at the time referred to it as “asbestos like”weird I know. When I could get decent amounts off, it would leave a circular open wound on the scalp. Took him to his pediatrician and he said Nizoral would likely clear it. If not he’d refer us to the dermatologist. Within a couple weeks, the Nizoral cleared it up. He has super thick curly hair. He using it 3 times a week. Hope that helps. Honestly, I’m no expert but it is identical to what he had as far as my eyes can see✌️
My daughter has this (6 month old). I’ve treated it with using dandruff shampoo (per her doctor). We just use regular head and shoulders but I’m sure there’s stronger ones out there. Then I put an ointment called green goo on it. Her “crusties” as we call them are gone. Seborrheic dermatitis.
That looks like sebhorrea, go to a doctor. In the meantime, you can use Head and Shoulders as it has antimycotic properties and its drugstore cheap.
Avoid leaving any humidity there, so shower and blowdry hour hair asap.
It will keep growing if you don't take care of it.

I had something that looked just like this (though it was less severe). Hypochlorous acid spray (like the tower or elements one) made it go away completely. With that said with it being this severe I agree with everyone else that you should probably go to a dermatologist/doctor, as something more serious could be going on (e.g., something wrong with your immune system).
Yeah mine was eczema too I’m pretty sure (because I have rosacea, and if you have one you’re more likely to have the other. Plus, it runs in my family).
Before trying that I had tried all these dandruff shampoos in case it was that, and moisturizing cream in case it was some kind of extreme dryness. I was absolutely shocked when hypochlorous acid made it disappear over a few days. That stuff is something else.
A few times, it’s started coming back ever so slightly, so I use it again and boom it’s gone before it really starts. It also helps with my rosacea. Love that stuff.
Are you drying your hair? I used to have a very similar looking problem. Tried all the medicated shampoos and things over the years but nothing worked. Finally cleared up when I realised it was my wet hair that was causing it. Started thoroughly blow drying my hair after each wash and it’s not been back for over 10 years
As others have said, first priority is a derm.
I just wanted to share my experience in that I had something that looked very similar and after taking prescribed treatment AND once I stopped using the shampoo - which I had been using for years - it abated and never returned. It’s like I suddenly became allergic to that shampoo. (Biolage for color treated hair)

this is a skin condition; not a haircare matter. these things can easily be treated, in most cases. i have a few small eczema spots, which disappear over night when i use the topical steroid that i was prescribed.
Do you have PCOS, by chance?
I discovered I had PCOS by going off birth control and one of the fun side effects I encountered while off the pill was mega scalp flakiness. Dandruff shampoo didn't do anything for me. Nothing did. Until I got back on BC and it's gone away again entirely.
So I suggest following other people's advice and seeing a derm first, but if that doesn't help, maybe it's something that seems completely unrelated.
Just to really drive it in - go see a dermatologist. If you don’t have a derm, go to your primary care provider at the very least before doing anything.
Like others have said, you can be prescribed a topical treatment that’ll take care of this, but best to learn what it is and how to best care for it.
A lot of the time it’s a multi-treatment matter or it will keep occurring. You want to get to the source of the issue which is often an autoimmune matter manifesting on the outside.
It looks like seborrheic dermatitis to me. SD is an overgrowth of a certain yeast that we all have on our skin. If you live near an ocean you could try an occasional swim because many people find that ocean water gives them some relief from SD. Some conditioner products can cause a flare up - try to avoid getting conditioner and styling products on your scalp/roots.
My advice - keep these pictures handy and book a doctor appointment. I can’t tell you the number of times I have opted to reschedule my appointments because the symptoms temporarily stopped for a few days. This way you’ll have evidence
I will also share my own personal journey with scalp care. Mine used to look like this and the chunks of dead skin were easily a 1/4 inch deep. I never really got any answers, and eventually when I approached my late 20s it went away completely. My hair at 35 is better and thicker than when I was a teenager. One tip I will share, is that castor oil proved to be very effective and I would rub a small amount onto the bald spots at night and the itch would be gone by morning.

I started using the Briogeo scalp revival with a silicone massaging brush & letting it sit in my hair while I wash my body.
I also sometimes use my nephews cradle cap shampoo & it works really well honestly.
Get a good rough scrub with the shampoo and then let it sit a few minutes before rinsing out. Don’t be afraid to double shampoo!
Also, seeing a dermatologist could be really beneficial.
This is for sure seborrheic dermatitis. I’d recommend researching shampoos for this type of dermatitis as well as contacting a dermatologist as they have prescribe medication that you can’t get OTC.
Seborrheic dermatitis. The only thing that'll really help will be coal tar shampoo. It smells terrible and when your scalp is raw from the itching and peeling it doesn't feel great but for me it controls it 100%.
Other shampoos like head and shoulders or selsen blue actually make the problem worse for me.
First suggestion would be to go to a dermatologist.
However, I had this issue for years (assuming it’s seborrheic dermatitis/psoriasis) and I couldn’t afford the the steroid product prescribed by my dermatologist so I had to trial and error a bunch of home remedies and shampoo brands.
What finally worked for me was just Dr. Eddie’s Happy Cappy Medicated Shampoo. You can find it in the baby section of Walmart/various drugstores or buy online. When you wash your hair, make sure to massage the shampoo onto your scalp with your finger tips (make sure you’re applying some pressure to really clean your scalp) for at least a minute or two, and then massage your scalp again as you rinse to get all the product off of skin and out of your hair. I don’t use any conditioner or other product, I just comb my hair with a thick tooth comb after I shower. If you need to use conditioner/hair oil, make sure it is only on the ends of your hair, keep it a good distance away from your scalp.
It took a couple of weeks before I even noticed a difference, and about a month before I noticed big improvement. Probably has to do with the skin cell turnover. Unfortunately most things won’t cure it overnight (except probably prescription steroid).
A lot of people also swear by T-Gel and Selsun Blue (didn’t work for me personally but everyone’s skin is different). If a product doesn’t improve your scalp after a full month of consistent use, switch to a new one.
Don’t go to bed with wet hair. Don’t pick or scratch your scalp (as tempting as it may be). You probably will need to wash your hair more frequently, especially while your scalp is clearing (every other day or every day depending on lifestyle). Also make sure you’re drinking plenty of water in general.
You can use a super thin layer of hydrocortisone cream on your hairline if your scalp if peeling/flaking in that area, but do this sparingly so you don’t build a tolerance.
If it’s a fungal infection you’ll probably need actual meds unfortunately.
Sorry for the long post! I see everyone suggesting going to a derm but I also know that’s not always realistic (location/insurance/finances) and wanted to weigh in with my own personal experience. Best of luck to you ❤️
I have seborrheic eczema which is severe on my scalp, I have to use a ketoconazole anti-fungal shampoo to prevent secondary infections which look just like this. Get an antifungal shampoo
Please go see a dermatologist!!!! A lot of times there’s just stuff going on with your body, and using products or not using products won’t help.
I was diagnosed with alopecia after visiting a dermatologist, then a little while later i went back because i was having a similar issue to what you’re dealing with. I had a biopsy done on my scalp and they then told me i had psoriasis. They’ll recommend you proper treatment.
If you self diagnose and treat yourself based on that, it could cause a lot more issues if that self diagnosis is wrong.
Dermatology, sure but probably your root problem is in your gut. Derms will treat the symptom but if the cause is not addressed, the symptoms will return. Something is out of balance. Consider going to a specialist that can look at you as a whole organism opposed to just broken parts.

This looks like my psoriasis on my scalp. My doctor prescribed me a cream I put on the spot and it goes away for a few months and then I put cream on again and the cycle continues.
Wow. I suggest white vinegar cut in half with water and spray in your hair after every shower. Then rinse. It will clear up.
Look it up: Because of its antiseptic properties, apple cider vinegar may help soothe the itching or irritation from psoriasis, especially on the scalp, according to the National Psoriasis Foundation. Those who support its use suggest applying the vinegar directly to the scalp several times a week.
Seborrheic dermatitis (aka dandruff). Apply some conditioner and massage to loosen the flakes. Then brush them out with a shower comb. Use anti-dandruff shampoo going forward.
Inkey list salicylic acid pre wash for an hour or so and/or walmart equate coal tar shampoo in the shower let sit for 5 minutes at least. I do both when my flakes get very bad
see a dermatologist. You will probably need something that if I remember right, tar shampoo. .y husban had that on his scalp just like yours. It really helped him.
There’s definitely more going on than just dandruff and I think this goes beyond just using Head & Shoulders. This looks almost exactly like my sister’s scalp, she has psoriasis and I had something similar as a kid, though that was due to a fungal infection. Regardless, get to a dermatologist ASAP because you don’t want to end up with a bald spot. 💔
Yeah that's definitely dermatology report, it very much reminds me of psoriasis or eczema, they can tell you what it is and give you something to treat it
Selsun blue the one with a red lable that says medicated worked for my son in only 3 days you leave it on while you shower and rinse it off last it also works for any fungal acne or skin issues
Psoriasis; differentials include severe Seb derm, tinea. But psoriasis most likely. Use some Neutrogena t-gel shampoo and see your family doctor for some topical steroid lotion/shampoo to settle underlying inflammation
Looks like seborrheic dermatitis - I have it too and use steroid lotion on it which calms it down but unfortunately if it is that, it’s a chronic condition and so there is no permanent cure.
Have you tried the Nizoral Anti-Dandruff Shampoo with the 1% Ketoconazole? Dark blue shampoo. The difference in this shampoo is it has an anti-fungal in it, not zinc. I use it when my psoriasis flares up on my head during stress and only shampoo that works.
Try rinsing with 50/50 vinegar & water. Wait a minute and rinse with water. Have water running because it is uncomfortable when it gets near your eyes. Not sure what this is, maybe dandruff shampoo could help also
So I have hair issues similar to your anti-dandriff shampoo, and maybe once or twice a week at most, I apply conditioner you may need more cause your hairs longer, but no other product. Also regularly change your shampoos and conditioner.

Can you share what those were? I have seborrheic dermatitis and my derm gave me a minoxidil spray to regrow hair. I never used it except for a week since I was worried about the side effects and I do have health scares so I stopped using it.
Same!!! I wasted so much money trying different shampoos but as soon as I got topical steroids from the derm it cleared up within 2 days!!

I recommend using Nizarol - it helped me a ton with my psoriasis, triggered by stress. I started by using it every time I showered (leaving it on for 3-5 mins). Once things were under control, I started using it once a week and then every few weeks. Any dandruff shampoo can dry out your hair, so I would say use a hair mask more than often. You can also use a glycolic acid exfoliater (even the ordinary one for $7 - seems recommend that) to help with some exfoliation maybe 1-2 times per month.
Oh so Nizoral actually helps with Psoriasis instead of just dandruff?? I'm gonna have my husband try that. He gets *really* bad Psoriasis on his head and face. The only thing that works is a special cream he gets from the doctor, but we can't use it in his hair. So he's been using a mixture of shampoo and baking soda. The shampoo and baking soda actually works really well but he has to use it at least every other day, or we start to see it coming back already. No matter how long he's been using it for.
Yeah, the active ingredient Ketoconazole, is what derms prescribe (at a higher %) for psoriasis!
He can ask the doctor for a scalp solution! I also have scalp psoriasis and am prescribed a steroid solution for it.

At the very least, you should switch to an anti-danduff shampoo that targets seborrheic dermatitis. This would be something like selsun blue or head & shoulders clinical (which use selenium sulfide as an active ingredient).
It’s def seborrheic dermatitis, my husband had this. You should go to your doctor but T-gel worked really well for myself (left it sit a white before you rinse)

Came to say same - and also don’t sit on it, because come to find out it can spread to your face, and even your ears. Have had a chronic flare up in both ears for months now (stress) and have a freaking bald spot in my eyebrow.

I used to exclusively use Tgel because it was the only thing that worked for me. My stylist had me try Lanza scalp balancing shampoo and conditioner. It works even better and doesn't smell like tar lol

Yes. Please don’t pick it. Those scales can hold bacteria, which can cause infection, creating damaged follicles and even scar tissue. There’s your bald patch. Also, you don’t want that stuff under your fingernails and then you go eat a sandwich. If you’re not going to the doctor, start with Selsun Blue or T-Gel and see what happens after using it a few times a week. If it doesn’t clear up then you absolutely need to because it possibly could be fungal, which can spread. Even if not, it’s not good for you. Plus, it’s just nasty. It smells like flopsweat and will always be present in your hair, even when you’re clean. I’ve seen clients with it and I usually recommend a dermatologist. Nine times out of ten, I can tell they won’t go and then I awkwardly have to explain that they can’t come back until we can confirm it’s not fungal or something. I don’t judge because it can happen to anyone, but others will, like at your job or on a date. It’s just best to get it sorted out quickly before it gets worse. Left unattended, it very possibly can and you’ll be kicking yourself about it.

Don’t pick it. I know, I know, I know, but go see someone. Most of these things don’t have actually long lasting hair loss. There are so many things that can make this happen so it’s best to see a doctor. If only yo dave you time and the chore of dealing with flakes and plaques.

Please listen and read carefully: This is only going to go away and be treated by a dermatologist. It is most likely plaque psoriasis. This will not go away with a cream, a shampoo or a home remedy as what is causing it is caused by your immune system and it can and will get worse and more painful. Please seek out a licensed dermatologist as soon as possible, as this could be the sign of something more serious happening to your body.
This was one of my main symptoms when I was little for my Psoriatic Arthritis. If the meds that are available now we're available then, I could've had a much easier time, and there's be much less damage now. Definitely see a dermatologist and rheumatologist ASAP.

Absolutely agree. I struggled with home “treatments” for years before I saw a dermatologist and after 1 appointment and prescription cream/shampoo it cleared up within a week. I started getting seborrheic dermatitis on my scalp and face as a kid and my family went to a general doctor and we were told it was ringworm so we “treated” it with otc fungal cream for a year. During that year it got worse and worse to the point where my skin constantly raw and bleeding, so we eventually went to a dermatologist who diagnosed it and prescribed a cream and shampoo. After one use there was immediate improvements and it cleared up within a week. The derm appointment was literally less than 15 min and could have solved years of suffering if we had went to the dermatologist first!
And I imagine it can cause hair loss if left untreated as well. I think it’s the same thing.

I have psoriasis…the dermatologist needs to test it. I have gone to dr that just diagnosed it by vision …do not let them do that. It took years to control. Psoriasis is an autoimmune disorder …I have gotten sick over the years and it spread to my entire body. They first told me it was dermatitis years of steroids and crap that does not work. Yours looks more on the dermatitis side…mine is just white and red under it. Those shampoos are good at clearing once in a while like once a week . That is it. They dry your hair out and coal tar…which is tgel…causes cancer. This is true information. I have to take an injection of skirizzi and my scalp still has it some but it’s is controlled. Nizoral is for anti fungal dandruff etc…just find out what it is before slapping crap on your head. I have had psoriasis for 30 years … you are Lucky they now have medication that works pretty well….they did not when I was a child. Going to the tanning bed once a week probably heals it the most though. UV lights do something to it…like tempo kill it. So I started tanning at a really young age…living on the beach was pretty much the best thing…along with prescription medicine. Nothing over the counter has ever worked for me. There is a good chance you do have dermatitis though and they have different scripts for that. They also have a new script for some types of eczema that supposedly clears it up amazingly fast.

Retired hairdresser here! Looks like seborrheic dermatitis to me. I'd definitely get it checked from a derm Dr to be safe! But I highly recommend using Nizoral. My hubs scalp used to look like that and Nizoral helped clear it dramatically!
You really need to see a doctor. They can give you a prescription shampoo that will resolve this very quickly. Seriously, you’ll be kicking yourself for not going earlier. Unrelated but I’m curious why you only use shampoo? Wouldn’t that just make your scalp and hair dryer and ultimately more greasy?

Eczema crew, checking in! You might be allergic to something in your shampoo. Or if you're extra lucky, you might even be allergic to something in your water. I'll echo the other comments to visit a dermatologist. I know that can take a while, so in the meantime, I'd switch to using Vanicream's Free and Clear shampoo. It has the fewest number of ingredients and is made to be as skin-friendly as possible. My derm recommended it to me when I first started by eczema \~journey\~
Or if you are extra extra lucky, you might be allergic to foods that cause psoriasis reactions in some people (like gluten - its me).
Or wven just the Vanicream bar or Sebamed bar. The shampoo didn't work for me, but the bars do.
If you tried using Selsun Blue or dandruff shampoo like head and shoulders and it’s still not working, you might have dermatitis or something similar. I have the same problem. The doctor recommended Nizoral and it worked for me. It’s over the counter anti-dandruff shampoo I think? You can get it at a pharmacy. But if that doesn’t work, they prescribe steroids for the scalp. I’d recommend going to the doctor and checking this stuff out.
Great recommendation! Just to piggyback off of this… I had something similar and my derm recommended Selsun Blue because it has a different active ingredient than Head and Shoulders. When you apply, let it sit for 5-10 mins approx 3x/week. It needs to sit on the scalp to kill the fungal growth. Nizoral will work the same way, too. I tried to keep it off of the ends of my hair because it’s drying. Doesn’t smell the greatest but it’ll do the trick. Also, dry your hair with a blow dryer. Keeping the scalp dry will keep overgrowth at bay. Might save you a co-pay the dermatologist… but if that doesn’t work, see a doc! Coconut oil, sleeping with wet/coated hair, scrubbing etc will make it worse.
Great tips! Yeah Selsun blue is really good too! It usually works for me as well. For some reason though, when I’m really stressed, it doesn’t do the trick completely :/ Also yes, OP don’t use coconut oil! I saw some people suggest that. It will most likely just make it worse and feed the fungal infection if there is one.
Yea I use Nitozal too, I had a 2% prescription ketoconizole shampoo from my derm but this one works for me too!

I really think you should go to a doctor or a dermatologist. You could even send in pictures via teladoc if you insurance covers virtual visits. Your doctor will be able to prescribe you something that clears up your current flare up and will help you with what products to use and what to avoid to prevent future flare ups.
My husband's scalp looks like this and has refused to go to the derm as recommended by the doctor. It gets itchy and he scratches at it so hard it leaves bloody furrows in his scalp. Sometimes I'll lightly scratch/peel it for him so he doesn't carve up his head like that. Really needs to just go to the derm as you should. It has only gotten worse over the years for him.

God only knows. I keep telling him if it's psoriasis it could be doing internal damage too and he just says he's fine and it doesn't affect him at all
Tell him he needs to do it. Point blank. You scratching the plaques if it is psoriasis likely makes it worse. He needs to do for himself and for you. If he has psoriasis then he feels the burning and discomfort. He needs to get this settled because with a lot of these things the longer you let it go the worst it gets. It’s important to get this under control. He will be able to feel so much better.

This kinda looks like pityriasis amiantacea. If you try to remove the build up, is it pulling your hair at the same time (is it stiking to the shaft of the hair near the scalp and won’t go away)? I had a similar condition occur to me a couple of years ago and I got rid of it using Nizoral once a week and alternating with Neutrogena T Gel. However what I had might be completely different from you, I never went to a dermatologist so I’m not sure what it was. As other people recommended, it would be better to consult a dermatologist since they could give an accurate diagnosis and prescription.
Seb derm or psoriasis. Get thee to a dermatologist! It’s probably easily treatable with the right prescription and instructions. My family are Seb derm folks. But I am not a doctor. You need one. No worries, ok?
Nizoral Anti-Dandruff shampoo is the BEST I have ever tried and they make multiple types that target this specific issue. Saved my scalp.
My 10 year old son had this. He’d shower and after I would try combing them out of his hair and scalp. The information I found at the time referred to it as “asbestos like”weird I know. When I could get decent amounts off, it would leave a circular open wound on the scalp. Took him to his pediatrician and he said Nizoral would likely clear it. If not he’d refer us to the dermatologist. Within a couple weeks, the Nizoral cleared it up. He has super thick curly hair. He using it 3 times a week. Hope that helps. Honestly, I’m no expert but it is identical to what he had as far as my eyes can see✌️
I had this for a long time in my early 20's, broke up with a shitty boyfriend, and it immediately cleared up.
My daughter has this (6 month old). I’ve treated it with using dandruff shampoo (per her doctor). We just use regular head and shoulders but I’m sure there’s stronger ones out there. Then I put an ointment called green goo on it. Her “crusties” as we call them are gone. Seborrheic dermatitis.

That looks like sebhorrea, go to a doctor. In the meantime, you can use Head and Shoulders as it has antimycotic properties and its drugstore cheap. Avoid leaving any humidity there, so shower and blowdry hour hair asap. It will keep growing if you don't take care of it. I had it as a kid, and it went away by itself. I will try tl find the shampoo I used along with Head and Shoulders and get back to this post.
I had something that looked just like this (though it was less severe). Hypochlorous acid spray (like the tower or elements one) made it go away completely. With that said with it being this severe I agree with everyone else that you should probably go to a dermatologist/doctor, as something more serious could be going on (e.g., something wrong with your immune system).

Yeah mine was eczema too I’m pretty sure (because I have rosacea, and if you have one you’re more likely to have the other. Plus, it runs in my family). Before trying that I had tried all these dandruff shampoos in case it was that, and moisturizing cream in case it was some kind of extreme dryness. I was absolutely shocked when hypochlorous acid made it disappear over a few days. That stuff is something else. A few times, it’s started coming back ever so slightly, so I use it again and boom it’s gone before it really starts. It also helps with my rosacea. Love that stuff.

Are you drying your hair? I used to have a very similar looking problem. Tried all the medicated shampoos and things over the years but nothing worked. Finally cleared up when I realised it was my wet hair that was causing it. Started thoroughly blow drying my hair after each wash and it’s not been back for over 10 years
That happened with me before. I have psoriasis. Use the T Gel shampoo it’ll get rid of it. Smells like moth balls but worth it I swear by it!!!

I just wanted to share my experience in that I had something that looked very similar and after taking prescribed treatment AND once I stopped using the shampoo - which I had been using for years - it abated and never returned. It’s like I suddenly became allergic to that shampoo. (Biolage for color treated hair) Please take care, OP. Stuff like that is no fun to go through.
See a doctor. If you can’t do that, try neutrogena T gel shampoo
this is a skin condition; not a haircare matter. these things can easily be treated, in most cases. i have a few small eczema spots, which disappear over night when i use the topical steroid that i was prescribed.
Do you have PCOS, by chance? I discovered I had PCOS by going off birth control and one of the fun side effects I encountered while off the pill was mega scalp flakiness. Dandruff shampoo didn't do anything for me. Nothing did. Until I got back on BC and it's gone away again entirely. So I suggest following other people's advice and seeing a derm first, but if that doesn't help, maybe it's something that seems completely unrelated.
Just to really drive it in - go see a dermatologist. If you don’t have a derm, go to your primary care provider at the very least before doing anything. Like others have said, you can be prescribed a topical treatment that’ll take care of this, but best to learn what it is and how to best care for it.
A lot of the time it’s a multi-treatment matter or it will keep occurring. You want to get to the source of the issue which is often an autoimmune matter manifesting on the outside.
It looks like seborrheic dermatitis to me. SD is an overgrowth of a certain yeast that we all have on our skin. If you live near an ocean you could try an occasional swim because many people find that ocean water gives them some relief from SD. Some conditioner products can cause a flare up - try to avoid getting conditioner and styling products on your scalp/roots.
My advice - keep these pictures handy and book a doctor appointment. I can’t tell you the number of times I have opted to reschedule my appointments because the symptoms temporarily stopped for a few days. This way you’ll have evidence I will also share my own personal journey with scalp care. Mine used to look like this and the chunks of dead skin were easily a 1/4 inch deep. I never really got any answers, and eventually when I approached my late 20s it went away completely. My hair at 35 is better and thicker than when I was a teenager. One tip I will share, is that castor oil proved to be very effective and I would rub a small amount onto the bald spots at night and the itch would be gone by morning.
This looks a lot like mine! I haven’t been diagnosed but I suspect I have scalp psoriasis, looks like you might too. I started using the Briogeo scalp revival with a silicone massaging brush & letting it sit in my hair while I wash my body. I also sometimes use my nephews cradle cap shampoo & it works really well honestly. Get a good rough scrub with the shampoo and then let it sit a few minutes before rinsing out. Don’t be afraid to double shampoo! Also, seeing a dermatologist could be really beneficial.
This is for sure seborrheic dermatitis. I’d recommend researching shampoos for this type of dermatitis as well as contacting a dermatologist as they have prescribe medication that you can’t get OTC.

Seborrheic dermatitis. The only thing that'll really help will be coal tar shampoo. It smells terrible and when your scalp is raw from the itching and peeling it doesn't feel great but for me it controls it 100%. Other shampoos like head and shoulders or selsen blue actually make the problem worse for me.
First suggestion would be to go to a dermatologist. However, I had this issue for years (assuming it’s seborrheic dermatitis/psoriasis) and I couldn’t afford the the steroid product prescribed by my dermatologist so I had to trial and error a bunch of home remedies and shampoo brands. What finally worked for me was just Dr. Eddie’s Happy Cappy Medicated Shampoo. You can find it in the baby section of Walmart/various drugstores or buy online. When you wash your hair, make sure to massage the shampoo onto your scalp with your finger tips (make sure you’re applying some pressure to really clean your scalp) for at least a minute or two, and then massage your scalp again as you rinse to get all the product off of skin and out of your hair. I don’t use any conditioner or other product, I just comb my hair with a thick tooth comb after I shower. If you need to use conditioner/hair oil, make sure it is only on the ends of your hair, keep it a good distance away from your scalp. It took a couple of weeks before I even noticed a difference, and about a month before I noticed big improvement. Probably has to do with the skin cell turnover. Unfortunately most things won’t cure it overnight (except probably prescription steroid). A lot of people also swear by T-Gel and Selsun Blue (didn’t work for me personally but everyone’s skin is different). If a product doesn’t improve your scalp after a full month of consistent use, switch to a new one. Don’t go to bed with wet hair. Don’t pick or scratch your scalp (as tempting as it may be). You probably will need to wash your hair more frequently, especially while your scalp is clearing (every other day or every day depending on lifestyle). Also make sure you’re drinking plenty of water in general. You can use a super thin layer of hydrocortisone cream on your hairline if your scalp if peeling/flaking in that area, but do this sparingly so you don’t build a tolerance. If it’s a fungal infection you’ll probably need actual meds unfortunately. Sorry for the long post! I see everyone suggesting going to a derm but I also know that’s not always realistic (location/insurance/finances) and wanted to weigh in with my own personal experience. Best of luck to you ❤️
Yeah go see a dermatologist. I had something going on with my scalp and ended up significantly bald . I found I had folliculitis and needed meds.
I have seborrheic eczema which is severe on my scalp, I have to use a ketoconazole anti-fungal shampoo to prevent secondary infections which look just like this. Get an antifungal shampoo
E45 scalp shampoo if you can find it.

Please go see a dermatologist!!!! A lot of times there’s just stuff going on with your body, and using products or not using products won’t help. I was diagnosed with alopecia after visiting a dermatologist, then a little while later i went back because i was having a similar issue to what you’re dealing with. I had a biopsy done on my scalp and they then told me i had psoriasis. They’ll recommend you proper treatment. If you self diagnose and treat yourself based on that, it could cause a lot more issues if that self diagnosis is wrong.

Dermatology, sure but probably your root problem is in your gut. Derms will treat the symptom but if the cause is not addressed, the symptoms will return. Something is out of balance. Consider going to a specialist that can look at you as a whole organism opposed to just broken parts.

Apart from what people already said do check out dr.dray on YouTube after going to a derm & getting it diagnosed & medicated as she gives extra tips that derms don't have time to give. There is some other good advice on yt too but it needs to be sorted well.

This looks like my psoriasis on my scalp. My doctor prescribed me a cream I put on the spot and it goes away for a few months and then I put cream on again and the cycle continues.
Wow. I suggest white vinegar cut in half with water and spray in your hair after every shower. Then rinse. It will clear up. Look it up: Because of its antiseptic properties, apple cider vinegar may help soothe the itching or irritation from psoriasis, especially on the scalp, according to the National Psoriasis Foundation. Those who support its use suggest applying the vinegar directly to the scalp several times a week.
Seborrheic dermatitis (aka dandruff). Apply some conditioner and massage to loosen the flakes. Then brush them out with a shower comb. Use anti-dandruff shampoo going forward.

There’s definitely more going on than just dandruff and I think this goes beyond just using Head & Shoulders. This looks almost exactly like my sister’s scalp, she has psoriasis and I had something similar as a kid, though that was due to a fungal infection. Regardless, get to a dermatologist ASAP because you don’t want to end up with a bald spot. 💔

I had the same thing. My dermatologist gave me a prescription for Nizoral. I also bought T-Gel and in 2-3 weeks it has cleared up a lot.
Selsun blue the one with a red lable that says medicated worked for my son in only 3 days you leave it on while you shower and rinse it off last it also works for any fungal acne or skin issues

Nizoral treatment might help. Only use the treatment. The shampoo is not that great. Coconut oil massages. Never pick it. No matter how tempting.
Most likely you'll get prescribed ketaconazole. You can get weaker versions OTC but it's worth the prescription. It'll change your life and it's cheap

Head and shoulders. My sons head looks like this when he gets an eczema flare up. Head and shoulders a few days a week clears it right up.
Dry your hair with dryer every time you bathe or swim!

Could be a WHOLE bunch of nastiness. Seborrheic dermatitis, ringworm, eczema, psoriasis, etc. - all of which you need to see a dermatologist for.
You need tar shampoo. DHL is my go to. It's smelly until you get used to it but it's worth it.
Get thee to a dermatologist, ASAP!

It looks fungal- seek out an anti-fungal shampoo treatment from a pharmacy and try that for a week or 2

Have you tried the Nizoral Anti-Dandruff Shampoo with the 1% Ketoconazole? Dark blue shampoo. The difference in this shampoo is it has an anti-fungal in it, not zinc. I use it when my psoriasis flares up on my head during stress and only shampoo that works.

Try rinsing with 50/50 vinegar & water. Wait a minute and rinse with water. Have water running because it is uncomfortable when it gets near your eyes. Not sure what this is, maybe dandruff shampoo could help also

So I have hair issues similar to your anti-dandriff shampoo, and maybe once or twice a week at most, I apply conditioner you may need more cause your hairs longer, but no other product. Also regularly change your shampoos and conditioner.
