But there's a lot of horror b movies compared to other genres, historically and especially today. For every 1 mainstream movie there's like 20 straight to DVD cash grabs. Obviously other genres have that too.
Then on the opinion side... personally i see movies like Conjuring, Sinister, Annabelle etc, as really terrible movies. Not the definition of B grade cus they are big budgets but still.
Hereditary is one of my fave films of all time and while there are a lot of films in the same category, i have not come across one that gave me the same unsettling feeling. Midsommar by same director is a good shout but you might have already seen that.
It is not like Hereditary, but a film that really gave me that same weird tense/unsettling feeling was The Invitation (the one from 2015). Don't watch the trailer, just press play!

The killing of a sacred deer ( very disturbing and unsettling)
The autopsy of jane doe ( the primary reason I find it scary is because of how graphic it is, because it's about dissection and autopsies. But the reveal is extremely shocking and unnerving
I'll go with my usual go-to of late for really creepy films (though perhaps not like 'Hereditary'), and that's 'Caveat.' Go in blind if you watch it though.
The Lodge (2019). I think it’s just as good as Hereditary, super underrated, same bleak vibes.
Benjamin Lee, writing for The Guardian, awarded the film four out of five stars, alternately praising it as "an accomplished beast: poking, prodding and teasing as it throws out potential twists before settling on the most devastating one of all. It’s not an entirely unpredictable revelation but it’s a smart, knowing and nasty way to go, taking the film to a place that’s both staggeringly grim and hopelessly sad."
Brian Tallerico of RogerEbert.com echoed a similar sentiment, deeming the film "a truly unsettling movie, the kind of horror film that rattles you on an almost subconscious level, making you more uncomfortable than going for cheap scares. Don’t ask questions or dissect the believability of the plot. Just check in."

It Follows, and Babadook get alot of love. Havnt seen them yet though.

Sort of my point. Altough I prefer those over other horror movies that are even more mediocre.

Lake Mungo. I don't get scared by movies much any more, but I watched that earlier this year and it instilled a deep sense of fear in me
