It's an old romanticism/gothic trope. The reasons are:
Memories are more beautiful than real people
Dead girls don't get old/ugly
Dead girls don't have opinions or interests
Give the protagonist a reasonable reason to be a recluse
The Bad Ending of the Otome Game does a good job of breaking the trope. The dying woman is testy and contemptuous of all the men attracted to her being beautiful and terminally ill.
You need to check out Lia's Bad Ending / Bad Ending of the Otome Game (first pic) then. The whole plot is about how Emilia's life is affected by the death of her best friend Ophelia. Makes fun of the "dying beauty" trope too while hitting you with serious emotions with a side of reverse harem drama.
The second one depressed me so much because I read she was actually going to die, so reading this story and their hopes and dreams knowing that it has to end actually hurt lol.

I think in ***Ascendance of a Bookworm****,* the disease of Isekai Protagonist *Rozemyne* isn't romanticized at all.
The depiction of her mana illness is very grounded and not easily + quickly solved.
And it doesn't serve as a plot device to attract hot white elite aristocratic or royal guys.
The only thing that makes me uncomfortable about this isekai story is *Myne X Ferdinand*....

Seeing this topic after the John Green video on Tuberculosis, makes me realize they really do be going for that TB looks (which was glamourized in the 1800s).
Though in manwha terms, I doubt they are thinking that deep lol. And just either want tragedy or comedy (in the cases where the FL/ML really isn't dying.)
I don't really like this trope much, bc most of the time they just get miraculously cured from an incurable disease. But Karina's Days, and Time limited villainess (idk if the names r correct) are the only ones I completed bc they die fr. If a character is dying, well in he title at least I'm expecting a sad manhwa man. Well, I will give i thought my time was up a chance tho, it seems interesting.

Unless something was recently added in I’m a Villainess, Can I Die? (The Duchess Has a Deathwish), I don’t believe the FL is physically sick, but mainly struggles with mental health and suicidal thoughts. Part of that actually does revolve around her relationship with her sister that died after battling illness her whole life, but I wouldn’t really say it falls into the terminally ill FL trope

The thing is, I don't like the time-limited terminally ill stuff. Too close to tragedy. That's not what I look for in Romance.
But I can usually accept sick. Provided it's done right, we get lots of moments of ML just protective and caring and it's delicious.
So I've made it a point to avoid the majority of these.
FYI: **I'm A Villainess, Can I Die** doesn't actually fit this trope. She is neither sick or terminally ill or anything in between. Well, unless you don't count mentally because >!she is very, very suicidal. Like she self terminated in her past life to begin with. So reincarnating was just such a bother. And FL isn't even angry. She's just so incredibly listless due to having no will to live.!< If her time is limited, its only because >!if people take their eyes off her for a second, she might end up jumping off another clocktower.!<
I actually find this premise rather fascinating.
I can relate to **I'm A Villainess, Can I Die?** because I have been struggling with depression for years.
But it does bother me that because she has the "Terrible Illness of the Heart" and is suicidal, she triggers the romantic feelings of the two handsome Male Leads...
Let me tell you, the Love of a Family or pretty Suitor/Partner is not enough to heal your mental illness.
I read I am a Villainess, can I die and I have to say it's a hidden gem. The FL is sick in the idea of suffering of extreme depression and I loved the story so much because it depicts REAL depression not like what 99% of the manhwa stories show when it comes to this topic. In most of the stories when a FL or ML is depressed they get cured like almost instantly if they meet the other lead of the story which is so silly and unrealistic. Here it shows the journey and shows as close as possible as it can get in such a story how depression real hits. If you guys want a bit of a dark story and a real good one at that I mega recommend this one.
Does Libertia count? She wasn’t actually dying was she? I thought it was all just a big misunderstanding.
We are still dealing with the Victorian obsession with tuberculosis
But the FL thinks that she doesn't have much time left and the ML is pretty much sick/dying due to the Rotting Curse that can be only cured by the Heroine herself.
Right and that's why it doesn't fit the trope. Characters in the story aren't attracted to her bc she's sick like the person says in the first pic you posted. No one but her think she's sick and her thinking that is just a plot device to empower her, leave her family and execute her bucket list.
Ok, but how does "time limited villainess belong in this trope. She is not sick. She is dead. There is a difference. Or are there any miracles and I just off the loop?

It broke my heart, not that I wished for others to due, but I really wished for her to be healed, she been through so much😭

In most of the stories when a FL or ML is depressed they get cured like almost instantly if they meet the other lead of the story which is so silly and unrealistic. Here it shows the journey and shows as close as possible as it can get in such a story how depression real hits. If you guys want a bit of a dark story and a real good one at that I mega recommend this one.

Tbf in "I thought my time was up", the ML has bo idea she's sick and the title suggests she's not dying at all.
But the FL thinks that she doesn't have much time left and the ML is pretty much sick/dying due to the Rotting Curse that can be only cured by the Heroine herself.
Right and that's why it doesn't fit the trope. Characters in the story aren't attracted to her bc she's sick like the person says in the first pic you posted. No one but her think she's sick and her thinking that is just a plot device to empower her, leave her family and execute her bucket list.
