Well, I'm pretty sure I completed the game multiple times and destroyed every camera, yet it still hasn't given me the achievement and is always stuck at the same progress...
you can't use the chapter select button either, except for selecting chamber 0. obviously make sure you don't have cheats on either. there is also a bug with portal where achievements aren't given, i think you had to delete some file.
Sometimes steam being offline can mean achievements you got didn't proc correctly. I'm missing a couple of achievements in Tales from the Borderlands despite the only achievements being for completing chapters, and having finished the game, because one of the times I played happened to be with steam Offline.
Back in the old days, you could throw a camera down the incinerator and it would open the door, but you don't kill your companion cube and you don't get the achievement. This has been fixed
Must have been a bug, a lot of achievement systems have bugs and Valve's is no exception.
Yeah, I still haven’t gotten the achievement for finding the old HEV plate in HL2, yet I picked it up every way possible and reloaded the level.
Perhaps that level is bugged in such a way that you need to start from a new save.
I haven’t seen the plate in Eli’s lab yet got the achievement which is sad - i would really like to see it

you have to do it in one sitting.
I have, multiple times, even having a guide next to me to make sure to get them all yet the progress doesn't go up at all.

There's a program that you gan use to give yourself achievements. I'd say use it if you've done it legit.
