-Client decided they no longer required our services into structural design and renovation of the cantilevered tensions supported platform.
-Client began modifications on RHS load supporting tension rod without consulting our original design or engineer who worked on the project.
-The resulting actions of the client caused catastrophic failure.
-Upon reviewing available evidence it was clear:
Client worked to removed a sizable area of the material about halfway up the supporting tension member.
At the time of work the platform supported by the tension member in question was fully loaded to the maximum design capacity.
The material loss was so severe that the tension member failed instantly, transferring the entire load to the secondary pin support at the platform level. This caused the pin member to yield elastically until plastic deformation was reached, at which point the support became a moment connection.
The pin plastic deformation then continued for a few seconds until the support could no longer maintain the moment connection. Causing failure of the secondary RHS support, and complete load path removal of the RHS of the structure. Remaining supports were not designed to handle the platform from the LHS, causing rotation under load about the bottom LHS pin connection.
Resulting in catastrophic failure of the entire platform, and probable death and/or serious injury to the client who was on the platform at the time.
Seems to me the cat was completing the remodel to level the platform without adequate shoring. The other side had it’s tension member shortened previously, possibly in the same manner, and the job was only halfway complete.
