Standing sedation is very common in the field for horses and operations like this. Unlike small animals (dogs and cats) who undergo induction and general anesthesia, that just is not a luxury horses have due to not having great vital parameter monitoring in the field and their large size. This fella likely received an alpha-2 agonist like Xylazine and even possibly a local anesthetic to the region such as lidocaine or bupivicaine. Field anesthesia can be tricky with horses as you want an amount that will provide adequate anesthesia, but not so much that you cause temporary limb paralysis and have them go down.
Horses are really heavy and can run very fast, my guess is that it was galloping and either didn't see a large branch poking out of a fallen dead tree (or something similar) , or maybe couldn't stop in time before running into something. Also possible that it tried to jump over a fallen tree on a trail or something and landed on the branch.
Could be, but could also be a straight piece of smooth brach, especially if it were a tree that had been sitting in water, essentially driftwood. Either way, point is, horse ran into something.
