Just stick in the main contact details of the company (such as their public number), or something like "no longer trading" if they're not in business any more.
The chance of them trying to speak to HR from a company 5+ years ago are basically zero.

I usually just put the main or local office for the company.
Just give the information as Contact Unknown, Human Recourses, Supermarket Address
I just put no longer in business for the former employers bit. No one question that so I'm sure it's fine.
