The bridge is under construction so might not be on maps. Here in the west most rivers are overseen and you get a lot of intel. Looks like a less developped country where this is taking place.
I wouldn't put it on the captain of the ship, at least not entirely. Looks like he's arriving at some sort of wharf or canal. He needs to have a pilot vessel guiding him along the path because it's impossible to steer a large ship in a small area like that.

Every collision is *always* on the captain. As a former ship driver, there is always a way to avoid hitting *anything*. Even if they lost all power, then it can probably be attributed to poor maintenance from the engineering department, which is still the captain's responsibility.
Edit: to clarify, other people can still have some degree of fault attributed to them as well, but at the end of the day, the ship is the captain's responsibility.

That was a dock they crashed into with a tug connected to them. They definitely would have had a pilot on board as well.

A captain is always responsible for his/her ship, regardless of tugs, pilots, weather, power, etc. I've *never* done a case study of a collision/allision that didn't end with a captain being fired.
