Yea the only on screen part shown a video game like CGI moment of Oliver swimming through water and then a shark following him, it cut to him bleeding on shore.

The bite was a shark from season 4 episode 9. It was off screen you see him swim back from the amazo and a shark swims up behind him. He then crawls out of the water bleeding with the bite.
I think they realized they didnt have the budget to actually show it and it look good, when they got around to it, but they had to justify the scar already being there in the present day.
Ask anyone who's worked with the guy. He's an awful person. He keeps his image up by being really good with fans. I know several people who worked arrow. He's not a good person.
I believe it was the Shark in season 4, it seems like an odd mark to me, but I'm pretty certain that is what it's meant to be

He had to swim back to the Amazo to get a map I think, and on his way back he got attacked by the shark, I think it was towards the end of the season
