From everything you've stated, it seems that there's a lot of good news. It's not organ-related, nor does it seem to be inflammatory based on what you're describing (plus that'd show in bloods/imaging if they were thinking that was the case), it doesn't appear to be anything from the nerve. All great things! In the span of five months you've tried a lot of different exercises, acupuncture, MFR, massage, breathing work - sounds like a lot of stuff. What did your life look like before this happened? Have you had to put things on hold because of this, and if so, what sort of activities have you had to stop or scale back since this happened?

My life before this looked pretty similar in the sense what I did but what changed is intensity. Now I just don’t go as hard as I would like or train as hard as I want to. Also like there is always a thought when is my back going to get better

It may be helpful to focus on what you can do, and what progress with that would like like. Say with squats, let's say right now you're doing box squats 3x-8-12 with a weight you feel you can do comfortably. Progress could look like doing more reps, adding more weight, lowering the height of the box (so more ROM), or doing it more frequently (twice a week instead of once a week, as an example). Over time, ideally, things get to be more normal. For some that happens soon, some may have more bumps in the road, and for some normal may be changed a bit. But if you try 15,000 different things, you're likely not going to hit the big things really well.

This may sound weird, but it could be pelvic floor related, if the pain is deep. Could be those deep muscles aggravating your QL bc its having to compensate. Esp if breathing exercises sometimes help.

Yeah I can see how that would be the reason! What do you recommend to do for pelvic floor? Maybe u can bring it up with my physio if I need to do some more screening

Core: McGill Big 3, Plank with opposite shoulder tap, Plank with Hip Extension Pelvic Stability: Side plank ABD with ER, Adduction plank Mobility: Thoracic mobility, Hip mobility (fot internal rotation) Strength: Single leg squat, Clamshells (with a band), Crab Walks You ve tried a lot of things. How about hip external mobility? Squat with band? Are you able to proper core bracing? Have you Hypo or Hyper -Lordosis? Can you recruit properly posterior and anterior tilt of the pelvis? How about glute bridges or similar exercises? Some things you wrote let me think about a issues related to a lack of stability, so learning proper core bracing and core strenghtening is what I d do. But you have already tried a lot of things. QL pain can be consider an SI joint issue, or a lumbar spine related issue,
