The document pretty clearly states it is due to his severe arthritis, and not a neurologic deficit, most stiffness in elderly people will be due to arthritis, literally everyone will get arthritis, and it's already started for anyone aged 20+ Moderate to severe spondylosis will definitely make a gait very "stiff", does not mean there is some neuromuscular rigidity pointing to some neurologic or cognitive deficit, the explanation appears more than sufficient given a detailed neuromuscular exam to more or less rule out things like parkinsons or a significant stroke or otherwise, these types of gaits will also look completely different Typically rigidity refers to overall rigidness (does not change if you examine patient with active or passive ROM), whereas "stiffness" is not specifically referring to rigidness, but more active stiffness due to typical arthritis, the stiffness is not necessarily going to be as strong with passive range of motion (while rigidity of parkinsons will be more apparent with passive ROM), and this is a pretty good way to differentiate between a neuromuscular source like in stroke or cerebral palsy leading to contractures or parkinsons and cogwheel rigidity versus something like joint stiffness due osteoarthritis or an autoimmune arthritis If he can draw a clock and remember 3 words he would score 5 on a mini cog, and even given his apparent memory difficulties, a moca he would probably clearly 20+ fairly easily, assuming some word forgetfulness and math difficulties, but I think he would probably guess the animals fairly well and recognize verbalized letters Take it as an estimate from a geriatrician who does this on a daily basis, many older patients are actually very capable of this, even into mid 80s, some even up to early 90s And no, I'm not American or care about this election, just enjoyed this document for how admirably detailed it is for a politician, like an essay with each segment written by a completely unique team of doctors lmao
