There were a lot of problems with that movie, but the fact that they were basically the same age made their scenes very odd. Jolie's bizarre accent didn't help.
In that movie's defense, the movie spans like two decades and it's easier to make a younger actor look older than an older actor look younger. Looking at House of the Dragon and how hard it was for audiences to connect some of those recasts, I can sympathize with Oliver Stone keeping Jolie as Olympias for the whole movie since there are three different actors for Alexander and the 15 year gap between Connor Paulo and Jolie is much more reasonable. Plus Jolie's scenes with the Farrell have her in a silver wig and old age makeup so they certainly don't look the same age unlike some of the other examples other people have mentioned.
When Love Actually came out Keira Knightly (18) was closer in age to the actor playing Liam Nesson's young son who's in elementary school (Thomas Brodie-Sangster, 13) than she is to her on-screen husband (Chiwetel Ejiofor, 26) or her husband's best friend that's also in love with her (Andrew Lincoln, 30).

I never watched the Fox TV series, but the actors playing Riggs and Murtaugh both looked younger than the movie counterparts, while Damon Wayans (Murtaugh) was 56 when the series started (vs 41 for Danny Glover) and Clayne Crawford (Riggs) was 38 (vs 31 for Mel Gibson). Speaking of Mel Gibson, he was 31 in the first film and looked older; I'm not sure how old Riggs was supposed to be, but he was special ops in Vietnam. In reality, Gibson was anywhere from 7 to 17 during the US's official involvement in the war.
In Community, Britta (Gillian Jacobs) is around 26 in the first season while Annie (Alison Brie) is 18. They even looked it, with Jacobs nailing it as this world weary young adult returning to community college while Alison Brie was convincing as this baby-faced innocent schoolgirl. IRL they’re only a couple months apart in age, with Brie (I think) being slightly older by a couple months.
