IF expected to wear for an extended period, one thing I'd do is a little research on the safety and health issues. I found one site that said that in general, 4 hours is the max insertion time recommended, but some people can't keep it in that long, and others can keep it in much longer. I'd buckle down and look closely at health issues so you don't risk getting damaged back there.
I've never gone a longtime, but when ordered to wear it for a while, especially when we're in public, I both lube it up well, and pair it with a butt plug holder (a hardness that keeps the butt plug in). That way it moves with me a bit, but the hardness keeps it from falling it. Just my solution for wearing in public

* find a shape that works for you (flanged on both sides to it sits well and won't slide either too far in or out)

My understanding is long term wear of butt plugs is not healthy, too much long term irritation can lead to bad stuff.

That combined with oil or silicone based lube (be careful with this depending on the toy you're using) can afford you some fairly comfortable long term wear. As for the feeling of messing your pants - that will go away with time.
