The majority of anthropomorphic sharks have both gills AND lungs. It would make sense given the logic needed to make anthro sharks work. The only people who'd be in trouble were those with feral characters that spend 100% of their time in the water, and you really don't see many of those. That and anyone with either a micro or micro character as the macro's would likely be injured or killed based on where they were sleeping and the micro's are at risk of falling or some pet or animal eating them.
I think the larger issue would be physics causing said macros to suffocate and collapse due to their own weight. Just because you're huge doesn't mean you're able to support tons and tons of pressure on material that was never meant to support that kind of mass. Dinosaurs could grow as large as they did because they had a unique bone structure. Unless the macro happened to think of that ahead of time and incorporate it into their characters background they'd be in trouble. The Devil-as they say-is in the details.
