Well,the newer DOOM games are brutal, but it's kinda cartoony at the same time... the newest mortal kombat is also very intense. Maybe Cyberpunk 2077 if you want NPC reactions?
Recently i played a free demo on steam for a game called Half Sword and the way the enemy reacts when you delimb them made me feel sorry for them, when they're very low on health they'll just whimper on the ground twitching and clutching to their wounds, when you disarm them, they look terrified for their life as they sometimes run full speed away from you to search for a new weapon, AND there's disembowelment, the body language in that game is the most realistic I've ever seen yet, however I don't believe the games has any facial expressions and also the other enemies don't really react to you ripping a person in half with a scythe right in front of them, it's like they just think "pfff anyone can do that".
There isn’t really a game that lets you actively torture people at a high quality level. For obvious reasons. It’s boring gameplay and there isn’t exactly a wide market for it. So effectively this kind of game doesn’t really exist. There are games with very high quality gore systems but they aren’t effectively torture.
1 million percent The Punisher from 2005.
They had to censor pretty much every interrogation/torture scene by making it black and white and dramatically cutting the camera angles before it was allowed to be released.

I'm not sure if there was ever an uncensored version of it released or if mods exist to "restore" these scenes but I remember it being a pretty big deal back when it was first announced.

Red dead redemption 2 lets you shoot peoples heads off.
Manhunt would best fit this description but it’s 20 years old at this point

Shadow Of Mordor is pretty savage
Shadow of Mordor scaring the enemy with your brutality is a built in mechanic Cyberpunk as well. Lot of dismemberment with a good sword or monowire.

I've been addicted to this game for a couple of weeks now, I spend hours playing it, very cool and realistic
