It's an attempt at nudging people to think about others and in this context to maybe offer a seat to someone less able to stand than you on the tube, it's been there for years
if you look at the link in the u/Quick_Doubt_5484's post, you can see the source and what it's supposed to look like. It's supposed to look like the tube map, but with people holding hands as the lines.
In cloth on the chair back, in a straight line, you can't see that — so it does look a bit odd.

Increasingly I seem to see adverts etc where the the person designing it thinks they are being really clever but if I look at something and I does not give me the information straight away I walk on.
I think the strategy is to design a poster etc that is bizzare / obscure it will make people investigate it thus achieving it's aim.
Maybe I am just too old for all this psycological symbolism.
At the risk of ire, I think it makes its point quite well..
It’s “six degrees of separation” (..on the width of a single seat; only two people fit).
It says: “Be kind to strangers, because they might be loved by someone who is loved by you.”
It’s: “love thy neighbour”
