Actually fettucine alfredo is named after Alfredo Di Lelio, who created it for his restaurant in Rome. It became a hit with American tourists who brought the dish back the US where it became very popular. The original version is different though because it is just butter and parmiggiano reggiano and butter prepared tableside, and doesnā€™t contain any cream like the Americanized version.

Fettuccine alfredo is known in italy as pasta al burro (pasta with butter). It is simple and dumb and considered hospital food. The alfredo variant is just specifically using fettuccine and a ton of butter. Basically it exists but doesn t have that name and is generally done slighlty differently. If i have to ingest a fuckton of calories that shit is bottom of the list since there an infinity of better recipes.... Now this is why if u ask italians most will tell u there s no such thing as fettuccine alfredo in italy, that name is alien to us, if u tell them the ingredients they ll understand.
