I have Nero in 0.3 and it's a tad scratchy too. The same thing with Automac in 0.5. I believe you can't have automatic lead feeding without at least a bit of scratchiness to it. What I do is I use both pencils just like any other pencil. Used like this, Nero is one of my favorite pencils.
I own the 0.2, I wouldn't say the auto feed works better on thinner leads, but it does make more sense. 0.2 is very hard to write with without breaking so it makes sense to leave it in the pipe. As for scratchiness, writing with 0.2 alone is scratchy, so to me it doesn't matter but I'm sure you would find it as scratchy if not more.
Nero in 0.2 and 0.3 feel more natural because super fine leads are already slightly scratchy in any other model of MP. So the rounded tip of the Nero doesn't feel all that different, and you get the benefit of the automatic feed. I would say the metal grip or even standard Orenz feels even smoother to use because the tip is not linked to the clutch, it just slides up to reveal more lead. And, yes, softer leads like 2B and up will make things feel even smoother.
Mine smoothed out after using it more. This was somewhere between 3-6 months of daily use though.
Still not smooth-smooth, but I don’t get much scratchiness unless my lead is getting on the short side.

The 0.2mm is probably even more scratchy. I returned the one that I purchased from Amazon right away.

My .2 works real smooth on super smooth paper. Try some high quality notebooks, I really think it was designed and tested for smooth paper.
