It's the mom jeans. The light acid wash denim clashes with the sleekness of the black leather loafers, black shirt, and accents on the wrists. It also seems to make the belt ride low and the pants ride high, which accentuates the goofy awkwardness (black shirt, black belt, acid blue stripe between them which seems high on the waist). The belt sits in the belt loops at the same height as the other fits, but only this one gives the appearance of awkward high-rise and ill-matching belt. Replace the pants and #4 would be great. Or, if you insist on those mom jeans, pair them with light color non-leather shoes and light color flowing/breezy top like linen. They would make the mom jeans blend rather than stand out like a sore thumb.

It's the socks, which need to match the black on the shoes/another part of the outfit, or you could wear them with 'no show'/short socks socks. Otherwise, it's pretty European styling, but with a more Americana/American western style embroidery on the polo. All the other looks are really good too. Edit: the sock thing also doesn't matter as much with those same shoes on look 3 because the shoes and socks use different colors to tie into the button down you are wearing.

I think with 4 you need a different pair of jeans. Dark wash, lower rise. Maybe even a more formal cut to go with those shiny loafers.
