It was in an early-middle / middle stage of infection, so the entire cyst wall hadn't yet been compromised and broken down - this is why there was some pus & bloody broken down cyst fragments early on, but as he moved on towards the end you saw much larger, thicker fragments of wall come out. When infected like that it can be very difficult to remove all the cyst wall fragments (not to mention patient pain / discomfort - infected tissue can't be "frozen" with local anaesthetic as well as healthy tissue), so any fragments left behind will eventually reform. So typically a procedure like this is done to clear the infection & inflammation, and several weeks/months down the line once the tissue is healthy, a second procedure is done to remove the entire cyst.
In this specific case, I don't think mrpopzit explicitly says either way whether or not it will need a second procedure (I fast-forwarded some parts) - it may have been caught early enough that draining & steroid injection and a few follow-up post-op checks & flushes might do the trick. However, it's really quite large & deep so I think a follow-up surgery a few months down the road is fairly inevitable.
