Maybe it’s a dumb question but, do these type of scissors have inside and outside blades? I see a lot of “stab and open” the tool like it is cutting that way too.

Nope, it's an actual surgical trick (I'd hesitate to call it a technique, because it's literally just opening a pair of scissors), where the surgeon is using the dull outer edges of the scissors to prise apart tissue. Blunt dissection is generally safer & less traumatic to surrounding tissue (particularly vessels & nerves) than sharp dissection. You see it done a lot in videos posted here on r/popping because it's particularly useful in derm procedures, for separating the various superficial planes & breaking down connective tissue..

The reason he did a straight cut was because he thought it was a lipoma. He admits that if he knew it were a cyst, he would have done an elliptical incision.
