Ostia Antica. A wonderfully preserved town with amazing mosaics and no crowds. About a 30-40 minute train ride from Rome. You could easily take a whole day and explore. Bring a lunch and some water.
I see that Ostia Antica is near the airport. If our plane leaves at 3:20PM/15:20, would you recommend checking out a little bit of Ostia in the morning on the way?
It is a must! And if you walk to the right once you get off the train and across the bridge you'll fine a great old castle you can walk around and the actual town of Ostia. It's wonderful. I would recommend getting lunch at one of the small restaurants there.
That particular town is called Ostia Antica and includes the excavations, the castle, a small medieval area next to the castle and a larger area of normal houses, shops and other modern stuff. As a neighborhood it's distinct from Ostia itself.
Absolutely LOVED the Baths of Caracalla. My new favorite attraction in Rome!
Pro tip - bring your own corded headphones for audio guides. They don't usually come with them & they're an additional charge. Also, the ones for sale are usually terrible quality.
The Appian Way (Appia Antica). A short bus ride away. Get off at Basilica San Sebastiano and see the catacombs, then walk down the Appia Antica to the Cafe where you can rent bicycles, bike down the cobbled road that feels like you've gone back in time 2000 years.
i would suggest to take an hour or so to go to MACRO, go up to the third floor, enter the room "musica da camera" and relax a little bit! this usually overlooked and negliged museum of contemporary art hosts musicians to put music in loop in this room, and right now until august Alva Noto curated the sound. the loop is short and repetitive but if you are in meditative things or experimental music then it is LIT (and you can visit this room and the rest of the museum for free)
take a look at tivoli: i love villa gregoriana (small but deep park that hosts waterfalls! you can find other villa, historical places, relatively good wine

We did a day trip to Sperlonga which was definitely a highlight of our trip. It was great escape from the hustle of Rome.
For something in Rome we did a tour with The Tipsy Tour which I really enjoyed. Also participated in a pasta/tiramisu class.
Centrale Montemartini. It is an old power plant that has been converted to a museum with mostly statues but all the old massive electrical equipment is still in place. Very cool, and very empty, at least when I visited

Enjoyed town of Lucca. Walled town. There is a music festival with concerts all through July. Top talent Ed Sherrin. Eric Clapton. Duran Duran were a few of the names I remember. We spent half day rented ebikes and absolutely enjoyed riding on the top of the wall which is like riding through a beautiful park. So relaxing and beautiful. Wished we were there for the entire day.

If you both are interested in World War II history — I find the two places to visit that are very powerful are the Fosse Ardeatine memorial within the city and the Abbey of Montecassino, which is about 90 minutes south of Rome. Both are striking places and convey the thoughtful and peace-focused ways the Italian people have set out to remember the horrors of that war.

I was gonna say this. Don't forget to stop by the beach on the way out. Only 3 stops away from ostia antica station on roma-lido line.

You really need most of the day to see it, but you might be able to squeeze a trip in the morning. Make sure you check what time they open.

Actually that's (even if it sounds contradictory) the modern Ostia Antica. Actual Ostia is one or more train stops away.

Consider a day trip to Tivoli to see the remains of Hadrians Villa and the gardens of Villa d’este, a UNESCO world heritage site 40 minutes from Rome.

About an hour, but the ticket also covers the Forum and Palatine Hill which generally take 2-3 hours to explore.
