It's on takedown which are kills/assit.
If you're wondering why he doesn't have 7 stacks (his total takedowns) it could be bc he gotten takedowns before finishing the item. (It only counts after he finishes the item)
"Scoring a takedown against an enemy champion within 3 seconds of damaging them"
so its 100% kills and assists only if you have damaged the enemy champ within 3 seconds

you are correct. Takedown always means kill or assist. If it would only proc on kills they would write kill in the description.
