Because in south American and Central American countries they have many different types of police from volunteer to paid officer with little to no education to paid officers who are essentially lawyers. There's so many volunteer officers because to be frank the people are tired of the cartels bullshit and corrupt officials not doing anything about it so they join to help fight them.

IIRC, a doctor can also make this diagnosis over a phone call if another medical professional can provide the doctor with vitals and observations.

The human body is a wild card. We've seen minor injuries and falls completely kill or paralyze a person for life. Then people can get shot 5-10+ times and still survive. People can take rifle rounds to the skull and recover whereas a .22 to the temple can still be lethal for suicide.
It really depends on what gets cut and severed. I'm no doctor but the older I get the less I feel capable of judging severity of injuries because everyone handles them so drastically different.
