Honestly as sad as it is one of you needs to give up the dog. When she’s dating someone new are you guys gonna still do this? In 4 years... think of the big picture
Maybe just say on a certain day of the week you may pick up the dog and drop it off later. Preferably a day when she at work so theres no in person contact if that's what you want.
I am sharing "custody" of our dog. For many reasons that I won't go into. But the most important part is cause we both love the dog very much.
I would have cut contact with her if not for the dog. So we keep contact minimal. Just arrange time to pick-up and drop-off. If dog is unwell, we will let each other know.
But I have to admit that seeing her when we do pass the dog to each other sometimes still affect me. So you need to think if you will be okay with that.

Our dog passed away in October 2021. So there is no more contact between us after that
