He probably got a hostile npc wandering across Sanctuary or something, and they got a lucky shot or two.
But don't you get notified when one of your settlements gets attacked? I didn't get anything and everything else there seems fine. Also, wouldn't there be any bodies from attackers? And how would they even get so far into the settlement, the whole perimeter is full of turrets.
You get notified for attack *events*, but not for combat taking place in your settlements. You can sometimes get, say, a raider chasing a radstag, radstag runs into Sanctuary, raider follows etc. You get this with Sunshine Tidings all the time because there's a raider spawn spot just outside the settlement.
He also appears to be unarmored. I’d suggest applying ballistic weave to settler clothes if you don’t want to armor them otherwise crits happen. Doesn’t need to be huge just enough to prevent a one shot kill from a lucky head crit

Also, I learnt over the years that I should never ever leave certain weapons in the hands of my settlers, else they end up killing themselves or their friends. (Like Flamers are completely banned,they always managed to set 3 other settlers on fire and then kill themselves).
That armor station looks like it intersects with an idle NPC animation spot. Maybe he couldn't spawn in correctly? The fact that he can die at all suggests mods. Mods?
