They're also all at least seven years old and old enough to know better.

Maybe it’s the parents who are stupid and he went in feet first and the parents couldn’t figure that out until after trying to just pull his head out for 5 minutes.
When we bought our home a few years ago the very first thing our then 10 year old son did was get his head stuck in the stair railing. Literally hadn’t brought a single box in yet and we were trying to get his head out of there.
