Rewards from events can be random but not quests, maybe you stumbled into an event whilst doing a quest or someone completed the "cop a squatter" event which usually rewards random items to whole server
Pretty sure this is the .44 you get from doing the Crane's Treasure questline after putting the token into the "Handgun" vending machine. Go to Duchess at the Wayward.

cop a squatter isnt an event its a daily quest
