Well it depends on what state you're in. In my state, I needed one therapist letter to get top surgery. Look up what's required in your state. The first step is to get your letter(s).

If you are planning on using insurance you should find out what pre-requisites are in place. I messaged my insurance through my on line account so that I had the requirements in writing. Ask about coverage for gender affirming mastectomy. If you have a PPO insurance you can go to any plastic surgeon that accepts your insurance and does top surgery. You can find surgeons here: https://www.topsurgery.net (I do believe the surgeons pay a fee to be listed so it is not a comprehensive list.) If you have an HMO type insurance you would need to get a referral from your PCP.

In my state (Oregon) I needed a referral letter from the doctor that manages my hormone therapy, and one from my therapist. They each have their own requirements for getting those letters.
